Temporary elevations in hs-troponin values—the challenge of interpretation

How is that hs-troponin can be elevated at the clinic but then back to normal in hospital?
The introduction of the high-sensitivity (hs) troponin assay has significantly improved diagnostics in cardiology. Because these tests are so sensitive, they can even measure traces of troponin that were once too small to be detected.
For general practitioners, however, that level of sensitivity itself poses a diagnostic challenge: it can happen that hs-troponin values found to be elevated at the clinic are no longer detectable or are only slightly elevated in hospital. This can often lead to uncertainty—both among medical personnel as well as among patients themselves.
Background: Natural troponin breakdown and temporary spikes
Troponin is continuously broken down in the body and has a half-life of roughly two hours. Values that are slightly elevated at first can fall below detection limits within one hour.

Elevated hs-troponin values are not necessarily an indication of acute myocardial infarction (type 1). The following are some of the many reasons why troponin levels may be temporarily elevated:
Potential reasons for temporarily elevated hs-troponin values:
- Type 2 myocardial infarction: A brief ischaemic event resulting from insufficient oxygen in the heart muscle; the condition may be due to the following:
- A fall or fainting spell
- Stress or powerful emotional response
- Major physical or emotional agitation
- Hypo- or hypertension, tachycardia, heart arrhythmias
- Hypertensive crisis (extremely high blood pressure)
- Anaemia
- Intense physical activity or training
- Heart failure: This can be accompanied by a chronic, low-level elevation in troponin values or by fluctuating values.
Case study: A patient whose hs-troponin levels are briefly elevated
A 62-year-old patient visits his GP due to non-specific chest pain. The hs-troponin assay returns a slightly elevated value.
The patient is admitted to hospital due to concerns he may have had a heart attack. While in hospital, however, his troponin values are back to normal.
Possible explanation:
The initial elevated value could be due to short-lived myocardial stress caused by a fall or brief fainting spell. During the time between the two measurements, his body broke down the troponin, which then fell below the detection threshold.

“hs-cTn values always need to be interpreted in light of the clinical context (symptoms, ECG, other laboratory parameters)!”
Conclusion
While the introduction of the hs-troponin test represents a major step forward in diagnostic cardiology, interpreting the results requires a nuanced approach. General practitioners should always look at the patient’s individual situation in order to avoid unnecessary concern or an incorrect diagnosis.
